Try shutting down your server, throw out the q3config.cfg file from the baseq3 folder and any mod folder, if you are running a mod. Then restart the server making sure you don't have ip banning on and remove any ip's that you added using "addIP". That should get you back in. If it doesn't, post back.

IP banning is the only game in town right now. There is some discussion going on at id apparently (see the current news about and id interview on q3 admin enhancement suggestions) regarding banning based in some way on the player's cd key, but that is not currently even promised. I have not delved into the admin enhancing mods for Q3 much yet, but in Q2 there were mods that would let you ban specific names, however that was not particularly usefull since the trouble maker could just come right back in under a different name.
What I have done in the past is to ban the subnet of the offending person and leave it that way for some period of time. I then remove the ban and see if they return. Usually they have moved on to bother someone else by then. With the advent of cable modem and dsl, more people now have static IP's, which can make banning easier.
You cannot stop a determined trouble maker from getting onto your server, but you can make it so difficult that unless they are truly psychotic, they will move on and leave your server alone.
The other thing you can do, if the player is being verbally abusive (foul language, threats, other harrassments), is to do a traceroute on their IP to find out who their provider is. Then send an email to abuse@whateverprovider.com detailing the time and ip address and the nature of the behavior. Harassment is against many ISP's user agreements and result in some disciplinary action from that end.
But most of the time a temporary subnet ban is enough to get rid of them. BTW, I will also track their ips over time starting with the first sign of trouble so that I can get a clearer idea of the actual range of ip addresses that they can log on with, thus allowing me to make the ban as focused as possible and not exclude any more players than is absolutely necessary. Fortunately, actuall bans are not needed that often. Usually a few kicks do the job and they get the message. Even just adding their current ip address, even if it is a dynamic one, to the ban list can drive them off because they have to disconnect from their internet connection and log back in to get a new ip address to skirt the ban. That takes a lot more effort and time on their part than it does for me to add their new ip to the ban list (two little commands on my part :-). After they tire of this and give up, I remove the bans :-)
